APPLICATION FOR NEW PASSPORT

GENERAL

Originals of the below listed documents, together with 2 sets of photocopies should be submitted with the application (originals will be returned after due verification)

ELIGIBILITY

Sri Lankan Nationals living in the Philippines

Requirements

  1. Dully filled passport application form (Forms are available at the Embassy)
  2. Current Passport
  3. Four passport size (4.5cm x 3.5cm) colour photographs it should be clear, good quality, fresh (current picture) without spectacles and with open ears, with white backgrounds
  4. Original birth certificate (translations are not accepted)
  5. If the applicant was born outside of Sri Lanka, the Citizenship Certificate
  6. If the applicant is a citizen by registration, the Citizenship Certificate
  7. Application fee along with previous N/M series passport Php 6,800.00
  8. Application fee along with previous J/K/L passport Php 15,900.00

Important Notice: 

Collection of Biometrics from Passport Applicants applying through Sri Lanka Diplomatic Missions Overseas

As a part of ongoing efforts to strengthen the integrity of the Passport issuance process, the Department of Immigration and Emigration of Sri Lanka has successfully introduced the mandatory collection of biometrics (fingerprints and digital face image) from Passport applicants and Automated Fingerprint Identification (AFIS) since 15th August 2015.

Further, the Department of Immigration and Emigration has now decided to extend the above system for Sri Lankans applying for new Passports via any Sri Lanka Diplomatic Mission overseaswith effect from 1st December 2017.

In this context, all Sri Lankans applying for new Passports through Sri Lanka Embassy in Manila with effect from 1st December 2017, will be required to provide their biometric information to the Department of Immigration and Emigration upon their first entry to Sri Lanka after obtaining their new Passports.

The necessary facilities for collection of biometrics are available at the Head Office of the Department of Immigration and Emigration, through its island wide branches and at the Bandaranaike International Airport.

ISSUANCE OF NEW PASSPORT FOR CHILDREN

Requirements

  1. Dully filled passport application form (Forms are available at the Embassy)
  2. Current passport
  3. Three passport size (4.5cm x 3.5cm) colour photographs it should be clear, good quality, fresh (current picture) without spectacles and with open ears, with white backgrounds
  4. Original birth certificate (translations are not accepted)
  5. If the applicant was born outside of Sri Lanka, the Citizenship Certificate or the payment receipt for applying of the citizenship certificate.
  6. If the applicant is a citizen by registration, the Citizenship Certificate
  7. Letter of consent signed by both parents.
  8. Death Certificate in case of either parent’s death, Divorce Certificate/Court decision together with the order of Custodianship of the child, if parents are divorced parents passports (bio pages and child inclusion pages)

Application fee below 16 years Php 5,300.00

ISSUANCE OF NEW PASSPORTS IN LIEU OF LOST PASSPORTS

Requirements

  1. Duly filled passport application form (Forms are available at the Embassy)
  2. Three passport size (4.5cm x 3.5cm) colour photographs white backgrounds
  3. A Police report obtained from a local Police Station
  4. A copy of the previous passport(N/M series passports)
  5. Original birth certificate (translations are not accepted)
  6. Application fee – Php 15,900.00 (applicant should produce a copy number of N/M series passport)
  7. If applicant is unable to produce the N/M series passport copy or number should be paid Php 20,400.00

Note: For applicants not holding a valid visa will be issued a Non Machine Readable Passport (NMRP) which will be valid only to return to Sri Lanka. Every applicant, including children requesting a NMRP should be presented for an interview with all relevant documents at the Consular Division of the Embassy of Sri Lanka in Manila.

Temporary Passports (NMRP) – Newly born children in the Philippines

Eligibility:

Children born to Sri Lankan parents in the Philippines
Birth should be registered at Sri Lankan Mission abroad (please see under registration of Birth for more details).

Requirements:

  1. Duly filled passport application form (Forms are available at the Embassy)
  2. Two passport size colour photographs

LIVE BIRTH CERTIFICATE ISSUED FROM PHILIPPINES AUTHORITIES

  1. Letter of consent signed by both parents to issue a separate passport to the minor.
  2. Payment receipt for Birth and Citizenship registration
  3. Application fee

TEMPORARY PASSPORTS (NMRP) – In lieu of a lost passport/expired

A temporary passport can be issued in lieu of a lost passport of a Sri Lankan National, only to travel back to Sri Lanka.

Requirements:

  1. Duly filled passport application form (Forms are available at the Embassy)
  2. Statement made by the applicant regarding the loss of passport
  3. Police report obtained from a local Police Station in the Philippines
  4. Copy of the lost passport
  5. Copy of the Birth Certificate and Sri Lanka National Identity Card
  6. If applicant produce previous passport (valid/expired) Php 2,300.00

If applicant’s previous passport lost Php 6,800.00

  1. Prior approval should be needed from the Controller General Immigration and Emigration in Colombo
  2. 4 colour photos in white background (3.5cm x 4.5cm)

AMENDMENTS OF PASSPORT – Inclusion of a child entry in parent’s passport

Eligibility:

Child entry inclusion in parent passport only can be done in “M”series Sri Lankan passports. If you have been issued a passport beginning with “N”series, please note that the child included passport will be cancelled upon arrival in Sri Lanka.

Requirements:

  1. Duly filled application form (Forms are available at the Embassy)
  2. Two colour photographs
  3. No objection letter signed by the father for the inclusion of the child in the mother’s passport or vice versa, if it is to be included in the father’s passport
  4. Birth registration payment receipt of child
  5. Application fee

RENEWAL OF PASSPORT

Eligibility:

Passports are valid for 5 years from the date of issuance can be renewed for a further period of 5 years. Total period of validity cannot exceed 10 years from the date of issuance.

Requirements:

  1. Duly filled application form (Forms are available at the Embassy)
  2. Current passport
  3. Application fee Php 150.00 per year
